1. Data Controller

Op. Dr. Emre Kaya, practicing at Mediastate Çekmeköy Hospital, Istanbul, Turkey, is the data controller as defined under the Turkish Personal Data Protection Law No. 6698 ("KVKK").

2. Purpose of Personal Data Processing
